Septorhinoplasty
All in one procedure
Septorhinoplasty involves surgery on the nasal septum and, at the same time, changing the shape of the external nose. This combines the functionally and medically necessary procedure (straightening of the nasal septum) with aesthetic changes to the external shape of the nose. Only in very specific cases is a change in the shape of the nose necessary for medical, i.e., functional reasons. If there is a hole in the nasal septum (septal perforation/septal defect), the nasal septum must usually be reconstructed before the shape of the nose can be changed. Only after complete healing should aesthetic correction of the external nose be considered. Only in cases of very small septal perforations can aesthetic correction of the external nose be performed at the same time.

Septorhinoplasty involves both functional correction of the nasal septum (septoplasty) and aesthetic correction of the nose (rhinoplasty). This type of surgery serves to improve nasal breathing and change the external appearance of the nose. If there is a hole in the nasal septum (septum perforation, septum defect), the focus is on the very difficult task of restoring the structure of the nasal septum. Only in selected cases, particularly those involving very small septum defects that have not been operated on previously, can aesthetic nose correction be performed at the same time.
Before septorhinoplasty, a clinical examination, a microscopic and/or endoscopic inspection of the nose, and, if necessary, a functional examination of nasal breathing are performed. In addition, imaging procedures, such as a digital volume tomogram (DVT), may be necessary, especially in cases of complex findings or holes in the nasal septum (septum perforation/septum defect).
Septorhinoplasty is a combined procedure in which both the nasal septum and the external shape of the nose are corrected—functionally, aesthetically, or reconstructively. The procedure usually follows these steps:
1. Anesthesia & Preparation
The operation is performed under general anesthesia. An anesthesiologist monitors you throughout the entire procedure. Before the procedure begins, the surgical team reviews all the details of the operation once again.
2. Surgical access
Depending on the goal and complexity of the operation, the nose is either:
operated on closed (without an external skin incision), but usually open (with a small incision on the bridge of the nose). Open access offers a better overview and greater precision, especially in complex cases.
3. Correction & Reconstruction
The nasal septum is straightened, stabilized, or—in the case of a hole (or perforation)—reconstructed. Cartilage from the ear or, less commonly, from the rib is usually used for this purpose. Depending on the agreed surgical goal, the shape of the nose is changed: for example, by reducing a hump on the nose, raising the tip of the nose, or restoring symmetrical nostrils.
4. Stabilization
After the operation, silicone splints or tamponades are inserted into the nose to stabilize the inside and support healing. An external nasal splint (or nasal cast) is applied to protect the shape during the first 14 days after the operation.
5. Wake-up phase & follow-up care
You will be monitored until you are fully awake. The procedure is performed on an inpatient basis by Dr. Stange. You will usually stay for 2-3 days for monitoring and care. After discharge, you will receive detailed instructions on aftercare and medication.
The initial healing phase lasts about 2–3 weeks. During this time, the septum foils, which serve to improve healing of the nasal mucosa, and the external nasal splint (aluminum splint or nasal cast) are removed. After that, you are usually fully socially acceptable. Most of the swelling and discoloration of the skin will have subsided by then. In the following 3–6 weeks, the internal swelling will then subside significantly, so that nasal breathing is usually considerably better. Healing progresses in small steps over the following months. After about 12 months, it can be assumed that healing after initial surgery on the nose is largely complete. Only then can the final result be clearly assessed.
